Hospital Patients

• Assistance with securing a physician/specialist appointment.

• Assistance with securing prescription medication

• Assistance with securing transportation to/from physician office

• Assistance with securing transportation to/from pharmacy

• Follow-up phone calls to assist in tracking vitals and preparing your own medical record to take on future physician appointments (e.g. weight, glucose levels, medication tracking, etc.).

• Home visits to assess fall risks and other risks that may lead to hospital re-admission.

• Assistance with filling out healthcare forms (i.e. Medi-Cal, Medicare)

Health Plan Members

We are excited to assist you with the following services:

• Assistance in navigate your health plan (e.g. Co-pay amounts; what services are covered; list of preventative services covered, etc.)

• Assistance with securing transportation to/from physician office

• Assistance with securing transportation to/from pharmacy

• Provide you a list of primary care physicians and specialists that are contracted with your plan.

• Make physician appointments on your behalf

• Make blood work appointments on your behalf

• Assistance with securing contracted specialists that are outside your immediate service area (i.e. Stanford, UC San Francisco, UC Davis)
